What if my Health Evaluation Form is not returned by July 1st?

All incoming students are requested to complete a health evaluation form prior to arrival on campus. In order to process all incoming forms and confirm immunization compliance, we ask that forms be returned to the SHC by July 1st. If you are not able to submit your completed health evaluation form by July 1st we ask that you complete it and return as soon as possible. Students that do not return their forms will be unable to register for Spring classes.

Can I get immunized for Meningitis at the SHC?

If you are unable to receive the meningitis vaccine through your primary care provider at home, you may receive the vaccine through the SHC. The vaccine cost $100. You will be given a receipt to submit to your insurance carrier for reimbursement. In order to receive the vaccine in the health center please submit a check (made payable to Saint Joseph's University) with the student’s name, SJU ID and cell phone number. We will contact students to receive their vaccine during the first two weeks of the semester. The vaccine is also available to be given during freshman orientation.

Who do I contact about special housing request due to a medical condition?

Students with specific housing request should contact the office of Residence Life at 610-660-1063. Documentation of their medical condition will be required.
