1. Point your browser to
   http://www.sju.edu/academics/cas/education/
2. Click on "To Receive ACT 48 Credits" link on the left hand side.
3. The “To Receive ACT 48 Credits” page will open up.
4. Please click on the ACT48 Form link here download the form.
   Please complete the entire form.

Please Note: You must have Your Professional ID# on the form. You may obtain your Professional ID from the PDE website at http://www.pde.state.pa.us

Your form will be returned if it is not filled out completely.

After the form is filled out in its entirety, please mail it to:
