Thesis Completion Form: To be completed when you are ready to register for credits involved in your thesis project. Please review this form with your thesis mentor.
Refer to the thesis checklist for help with this process. You will also need to complete a title page for your thesis (see form below) and complete the library bindery form.

Bindery—Library Form: Must be submitted with your signed and completed thesis to the program's administrative assistant, Heather A. Foster (see 'Contact Us' for more information). The Library Bindery form and thesis must be completed and submitted before a diploma will be given. However, while thesis binding is required of all students in the program, this process may take up to three weeks, and is not required to be completed before students receive their diplomas.
