A feedback form can be a great way of receiving comments or soliciting information about your website. It allows visitors to communicate with you in by filling out a feedback form and you to receive their feedback by email. If you are unfamiliar with feedback forms, please see this sample.

By following the steps below, you can generate the necessary HTML to have your own feedback form for inclusion on your website.

Step 1: Who should the form submissions be sent to? Enter a valid SJU email address below. If you need to use a non-SJU email address, this will require special server setup; please write to the SJU web administrator at webadmin@sju.edu for assistance.

Step 2: What should the title of your feedback page be? (e.g. "Send us your comments!").

Step 3: Should the email address of the sender be required for him/her to submit their feedback? (A valid reason to say 'yes' is if you need a way to get back to the person by email. If you simply want to allow visitors the option of leaving comments anonymously, select 'no' so that it is not required.)

Step 4: Do you want blank fields printed? (A valid reason to say 'yes' is if you would like to see that they omitted entering their phone number -- a blank Phone number: field would appear in your email.)

Step 5: Do you want an "environment report" sent with the email?
Something similar to this would get tacked on every email:

Step 6: Verify the above information is correct. Then proceed to click "Submit" below, after which you will get a page full of HTML. Cut and paste that HTML into your webpage where you would like this form to appear. Feel free to adjust it to make it look the way you want, such as adding extra fields to the form.
