VANITY PLATE ONLY

Complete the MV-904SP Form and mail back to Saint Joseph's University, along with two separate checks: one to "Saint Joseph's University" for $50 and one to "PennDot" for $34 to the address below.

Please note the fee comprises the following:

• State charge $34

• Donation to the Saint Joseph's Fund $50

• You will receive your plate directly from PennDOT. Please allow 6-8 weeks for delivery.

VANITY PLATE WITH PERSONALIZATION

If you would like to personalize your SJU plate, read the guidelines on the second page of the application before completing.

Replacement Plate – Peeling (Next Number in Series)

If your current plate is peeling and you are okay with receiving the next available plate number, complete Form MV-46. There is no charge for this replacement.

Replacement Plate – Peeling (Same Configuration)

If your plate is peeling and you would like to keep the same plate configuration, complete Form MV-46 and submit it with a $136 check.

Replacement Plate – Peeling (Previously Personalized)

If your plate is peeling and was previously personalized, complete Form MV-46. There is no charge to receive the same configuration.

Liberty Bell Plate – Next Number in Series

If your current plate is in good condition and you would like to switch to the Liberty Bell plate with the next available number, complete Form MV-44LB and submit it with a $14 check.

Liberty Bell Plate – Same Configuration

If your plate is in good condition and you would like the Liberty Bell plate with the same configuration, complete Form MV-44LB and submit it with a $136 check.

Liberty Bell Plate – Previously Personalized Configuration

If your plate is in good condition and you would like the Liberty Bell plate using a previously personalized configuration, complete Form MV-44LB. There is no charge to retain the same configuration, as required by law.
