New Student Requirements
Hawk Hill and University City First-Year & Transfer Student Requirements
All incoming currently enrolled, full-time, degree seeking, on-ground domestic Hawk Hill and University City undergraduate students, and all incoming currently enrolled, full-time, degree seeking, on ground domestic Clinical Health Professional graduate students are required to submit a Health Packet.
Forms must be submitted to the Health Portal by August 1st for fall semester and December 1st for spring semester. Students that do not submit their forms will be placed on a health hold and will be unable to register for classes.
There are three components of the Health Packet that must be completed:
1) Physical Examination Form, to be printed and brought to your appointment for your Healthcare Provider to complete. This form must be uploaded via the Document Upload function.
2) Health Evaluation Form, to be completed via the Student Health Portal.
3) Immunization Record, to be completed via the Student Health Portal.
Other University Requirements
Students in clinical and/or experiential learning programs should refer to their programs and clinical site coordinators to ensure they have met any additional requirements and have submitted those requirements to the appropriate office/portal. Failure to meet specific vaccination requirements of a clinical site for a given rotation may delay program completion.
Hawk Hill and University City International Student Requirements
All international students are required to submit the Tuberculosis-TB-Screening-Questionnaire, found on the Health Portal.

Lancaster First-Year & Transfer Student Requirements
*Coming soon! All incoming currently enrolled, full-time, degree seeking, on-ground Lancaster undergraduate students are required to submit the Immunization Record-Lancaster. When the form is available, students can access the form at the Student Health Portal: https://sju.studenthealthportal.com/Forms and login using your SJU credentials.

Health Insurance Requirement
Action will be required to waive or enroll in the Student Health Insurance Plan, starting in early summer. Eligible students will receive an email via their SJU email address when the waiver and enrollment forms are available online. More information about the School sponsored health insurance will be available in June. Please continue to check the Student Health Center website, Policies and Forms, and www.universityhealthplans.com/SJU.
The following degree-seeking students from Hawk Hill and University City must maintain active health insurance while attending Saint Joseph’s University. *Not applicable to Lancaster students at this time.
Students registered for at least six credits, with at least 50% of credits in person
Students registered for one or more credit hours, with at least 50% of credits in person
International students
PhD & EdD students
Students registered for the following credits with no minimum in person credit requirement
Domestic student-athletes registered for at least six credits
International student-athletes registered for one or more credit hours

SHIP Highlights
The SHIP is an ACA-compliant plan insured by UnitedHealthcare Student Resources (UHCSR) and includes access to United's national PPO network. Additional plan information can be found on the Student Health Insurance Plan (SHIP) website.
Enrollment Process
Students who do not submit an approved waiver by the deadline will be automatically enrolled and charged for the Student Health Insurance Plan (SHIP). Students should submit an enrollment form as soon as possible to avoid difficulties accessing their health insurance benefits.
Approximately four business days after the enrollment form is submitted, the student can access the insurance ID card via the Student Health Insurance Plan (SHIP) website. If an enrollment or waiver form is not submitted by the deadline, students are enrolled in the SHIP retroactively to the policy effective date and are responsible for the insurance premium.
Waiver Process
Students with comparable health insurance may submit a waiver form by September 6, 2024 to opt-out. To be considered comparable, your plan must:
Be filed and approved in the US and compliant with the Affordable Care Act (ACA)
Provide comprehensive, non-emergency benefits in the geographical area surrounding the school
Waiver forms may be reviewed, and if the plan does not meet the requirements, students will be notified that their waiver form is not accepted. Please refer to the waiver form for additional information about the waiver requirements and comparable plans.
Eligibility
Students will be retroactively terminated and refunded (pending a claim review) if they do not attend classes or fall below the minimum credit limit or on-campus credit requirement within the initial 31 days of classes.
Early Termination
Once the student meets eligibility for the first 31 days, the student remains enrolled for the entire policy period. Mid-year terminations are not permitted. However, if a student enrolled in annual coverage does not return to SJU for the spring semester, the student may request termination of the spring/summer coverage period by submitting the spring termination form by January 20, 2025.
Qualifying Life Event
Students who waived the SHIP may not voluntarily enroll during the policy year. However, students may qualify for special enrollment rights if they have a qualifying life event, such as losing other coverage. The qualifying life event enrollment form requires supporting documentation and payment and must be submitted within 31 days of losing other coverage.
